Protesan R is the middle rung of Nichinichi Pharmaceutical's FK-23 ladder: one trillion heat-treated lactic acid bacteria in a 1.2 g sachet, sixty-two sachets to the box. FK-23 is a human-origin Enterococcus faecalis strain that Nichinichi heat-treats and concentrates, so what you take is bacterial cell material rather than a live culture.
The previous version of this page said one trillion per package. It is per sachet, and there are sixty-two of them. The card also drops the word "drug", which the old copy used five times about a product that is a food.

Per sachet. Nichinichi states 1兆 per 1.2 g sachet, and the box holds sixty-two of them. The previous version of this page said per package, which is the same mistake the Protesan B page was making in the other direction.
Should I buy B or R?
Compare per sachet. B gives 300 billion in 1.0 g, R gives 1 trillion in 1.2 g, S gives 4 trillion in 1.5 g. R is roughly three times B for a sachet fractionally larger; whether you need three times as much is a different question, and not one a shop should answer for you.
Why are there no calories listed?
Because Nichinichi publishes the nutrition panel for the B sachet and not for the R sachet on its consumer page. Scaling the B figures by 1.2 would have produced numbers that look precise and are invented. When the maker does not print it, neither do we.
Are the bacteria alive?
No. FK-23 is heat-treated, which is why the box needs no refrigeration. It is a postbiotic rather than a live probiotic.
Does it need to be taken with food?
No required timing. Nichinichi's instruction is one sachet a day with water; the rest is whatever you will remember to do.
